For identifying information, please enter the following:
Enter your 9 digit Participant Number (located next to your name and address on the form you received in the mail). Click on the "Questionnaire Example" to help you locate your participant number. You will need to enter this participant number each time you log in.
Enter the first three letters of your last name.
Enter your Date of Birth. Please enter the birth year as four digits (i.e. 1956)
If you are completing your Juror Qualification Questionnaire for the first time, it will take approximately 10 minutes to complete the Questionnaire. Your responses will not be retained if you exit before completion of the entire Questionnaire.
If you are checking on your Status or Reporting Instructions, please login. Once you are logged in, click on the tab labeled Status / Reporting Instructions. If qualified, you must check your reporting instructions after 7:00 p.m. the day before the report date on your summons. Jurors who live 80 miles or more (one-way) from the courthouse must check after 2:00 p.m. the day before they are to report. It is important to check your reporting instructions because not all jurors will be required to report.If you need a Certificate of Jury Service, please login.
